Iowa HCBS Supported Community Living (SCL) and Brain Injury (BI) Waiver services tailored to your needs
Iowa HCBS Supported Community Living & Brain Injury Waiver Services
We provide Iowa Medicaid HCBS Supported Community Living (SCL) services under both the Intellectual Disability (ID) Waiver and the Brain Injury (BI) Waiver. Services are delivered in individuals' homes, family homes, and community-based residential settings — 1 to 24 hours per day based on each member's assessed needs and individualized Person-Centered Service Plan (PCSP), consistent with Iowa Admin. Code 441. Our Direct Support Professionals receive specialized Brain Injury training to support individuals with traumatic and acquired brain injuries.
Individual Home Setting
SCL services delivered in the member's own home or family home — supporting daily living, skills development, and community access while the individual maintains their personal living arrangement.
Group Home Setting
Residential-Based SCL (RBSCL) in a licensed group home — providing shared community-based residential living with individualized support, consistent staffing, and a homelike environment.
Personal & Home Skills Training
Supporting members in developing essential daily living skills including cooking, cleaning, budgeting, and personal hygiene to promote independence at home.
Community Skills Training
Assisting members in accessing community resources, social activities, shopping, and public transportation to encourage full community participation and integration.
Individual Advocacy & Life Skills
Empowering members to set personal goals, develop self-advocacy skills, make informed decisions, and actively direct their own care and daily lives.
Medication Support
Providing trained assistance with medication reminders and administration support to ensure members maintain their health and follow prescribed treatment plans safely.
Transportation Support
Helping members access transportation for medical appointments, community activities, shopping, and other essential needs to maintain independence and community involvement.
Personal Environment & Wellness
Promoting physical, emotional, and mental well-being through individualized care planning, personal environment support, and treatment services as defined in Iowa's SCL framework.
Behavioral Support & De-escalation
Our DSPs are trained in evidence-based behavioral support strategies and de-escalation techniques to help members manage challenging behaviors and maintain a safe, supportive environment.
Brain Injury Support Services
Specialized support for individuals under the Iowa HCBS Brain Injury (BI) Waiver. Our staff receive dedicated Brain Injury training — including Iowa HCBS BI Modules 1 & 2 — to support individuals with traumatic and acquired brain injuries in achieving independence and community integration.

Admissions & Referrals
Simple steps to begin receiving Iowa HCBS SCL services
We provide Iowa Medicaid HCBS Supported Community Living (SCL) services in individuals' homes, family homes, and community-based residential settings. Our team works closely with you, your family, and your Iowa HHS case manager to determine eligibility and develop a plan that fits your needs and goals.
Eligibility is determined by Iowa HHS. If you are not yet enrolled in an HCBS waiver, contact your local Iowa HHS office or call Iowa Medicaid Member Services to begin.

Your Iowa HHS case manager can verify your eligibility and authorize SCL in your Person-Centered Service Plan (PCSP).

Referrals are welcome from individuals themselves, family members, healthcare providers, or Iowa HHS case managers. We'll review your PCSP and assessed service needs.

We collaborate with you, your family, guardian, and case manager to build a support plan aligned with your PCSP goals.

What's the difference between individual home and group home services?
Individual home SCL is delivered in the member's own home or family home. Group home services are delivered in a licensed community-based residential setting with consistent staffing and a homelike environment. Our team can help guide you to the right fit.

How do I add SCL to my service plan?
Work with your Iowa HHS case manager to have SCL authorized in your PCSP. Once authorized, your case manager can refer you to a certified provider like A-Z Care Unlimited.
What are my rights as a member?
As a member of A-Z Care Unlimited, you have the right to be treated with dignity and respect, make choices about your daily activities and services, live free from abuse or exploitation, access your community, and have your personal information kept private and confidential in accordance with HIPAA and Iowa state law. You will be informed of your rights at the time of admission and may request a review of your rights at any time.
What if I have a complaint or disagree with a decision about my services?
A-Z Care Unlimited has a formal Appeals and Grievances process. You, your legal representative, or an authorized person on your behalf may submit a complaint in writing, verbally by phone, in person, or through an authorized representative — within 30 days of the incident. We will acknowledge your complaint within 5 business days and provide a written resolution within 30 calendar days. If you are not satisfied, you may request a second-level review. You may also contact the Iowa Medicaid Enterprise Ombudsman or the Department of Inspections and Appeals for external review.
How does A-Z Care Unlimited protect my privacy and confidentiality?
We are fully committed to protecting your personal and health information. All records are maintained securely in compliance with HIPAA and Iowa state privacy laws. Access to your records is restricted to authorized personnel only. You have the right to privacy in your living space and in your health decisions at all times.
Does A-Z Care Unlimited serve individuals under the Brain Injury (BI) Waiver?
Yes. In addition to the Intellectual Disability (ID) Waiver, A-Z Care Unlimited provides services under the Iowa HCBS Brain Injury (BI) Waiver. Our Direct Support Professionals receive specialized training — including Iowa HCBS Brain Injury Modules 1 & 2 — to support individuals with traumatic and acquired brain injuries in their homes and communities.
Is A-Z Care Unlimited an Iowa Medicaid certified provider?
Yes. A-Z Care Unlimited is certified under Iowa Administrative Code 441 to provide HCBS Supported Community Living services. We comply with all Iowa HHS quality oversight, incident reporting, and HCBS settings requirements.
